These are the documents that you will need to have filled out for your child to be able to participate in Little League. 
We need "wet" Signatures on the following documents- Meaning we need the actual document signed, and can not accept copies.

If your child has had a school enrollment form filled out in the past, we only need it done once unless you have moved schools
